As part of membership with ACN, you have access to a variety of unique services and resources.  We are spotlighting two of these services this month. 

Physician Friend of Family (PFOF) is a network of retired physicians available to answer questions you and your family have regarding issues arising out of a medical crisis or ongoing medical /social concerns associated with end-of-life care. Sometimes complex medical care involves multiple physicians, hospitals, care facilities with lots of decisions to be made among competing choices. Having someone available to review the whole picture and then to provide you an additional perspective on care options can provide support when you need it. Our experience shows our physicians help validate the choices you are making or help you identify other options that may work better for you.  Sometimes, it’s just nice to have a neutral expert at your side to mediate conflicting options presented by family members.  These retired physicians will help with communication between and among family members regarding disease trajectory and options for care. PFOF’s are not medical advocates; but expert resources on your side.

Medication therapy management (MTM) is medical support provided by pharmacists whose aim is to optimize drug therapy and improve therapeutic outcomes for patients. Services are provided by faculty from the UC San Diego Skaggs School of Pharmacy and Pharmaceutical Sciences. These services help you to navigate the complex pharmacy system with the aid of a Pharmacy Coach to answer standard medication or pharmacy related questions.  They are meant to optimize the use of medications to achieve safe, effective and the most cost-effective results. 

MTM services are useful for individuals who have:

• High prescription drug costs
• New or multiple diagnoses or conditions
• Recently been discharged from a hospital
• Changed their medications within the past 2-3 months
• More than one doctor who prescribes their medications
• More than one pharmacy that fills their prescriptions
• Medications that require laboratory monitoring

Because we believe so strongly in these services, they are offered to all.  However, members of ACN received preferred pricing.
